Thank you for your interest in Rosenberg & Sprovach. In order to better serve you, the attached questionnaire has been created. We ask that you respond to each question so that we can evaluate whether or not this firm can be of service to you.

Once we receive the completed questionnaire, preferably by fax or e-mail, you will be promptly notified as to whether an office consultation will be scheduled. In situations where we do not feel that this firm can be of assistance, we will do our best to provide you with names of attorneys who may be able to assist you with your particular problem.

The fee for the initial consultation is $250.00. The initial consultation fee is generally collected at the initial meeting. A consultation typically lasts for approximately one hour, however there are no additional charges if the consultation runs over. During the consultation we will identify all legal issues involved with your matter and make a recommendation as to the proper manner in which to proceed. Fees for further services are discussed during the consultation as they depend on the particularities of any given case.

Rosenberg & Sprovach generally confines its practice to the representation of individuals as opposed to companies, however there are some exceptions to this rule. Although a conflict of interest is unusual, from time to time some issues do arise. If in reviewing the first part of your questionnaire, a conflict check reveals that we will be unable to assist you, the questionnaire will be immediately returned without examination of the balance of it.

If your time concerns are urgent, please note that in the appropriate portion of the questionnaire. It is important that you understand, however, that this questionnaire is to be used exclusively for the purpose of evaluating whether or not the potential exists to form a attorney client relationship and no agreement is made to assume any representation until expressly set forth in writing.
